I would guess put the Image code (after uploading it) before you type the name of the forum. Try that and see if that will work, make sure that the image wont be too big.

To put small icons or images under the forum name part like on that site:

Admin CP > Forum Admin > Management > Edit (desired forum) >


What you need to post is something like this:

<img src="http://www.filehive.com/files/1025/firefox-title.png" alt="Image Hosted by FileHive.com">
With the "<img" tags, which will be an option to copy when you upload a pic to most image hosts. It's labeled "Hotlink your image on your personal website or Auction" on filehive.
