I assume you are talking about Enable account activation under Configuration? It depends on what you have it set on. If it is set on 'User', I assume a validation email will be sent out to the user when they register. And if it is set on 'Admin', you will probably have to activate all accounts manually. I am pretty sure that is what it means, I could be wrong though.
